Accepted Insurance

Open Door Health provides high quality services to everyone who needs them, without regard to a person’s insurance status or ability to pay. Everyone can get care at Open Door Health, and no one will be turned away because of their insurance or financial status. Our providers are in-network with many insurance plans, in addition to Medicaid and Medicare.

See below for a list of insurance carriers with whom we are in network. We recommend checking with your insurance provider to make sure your specific plan covers services you may receive from us. If your insurance provider is not on the list, or if your specific plan does not cover services received at our facility, you may be charged out-of-network fees or be subject to higher deductibles.

Sliding Fee Scale

Open Door Health offers financial assistance for healthcare and other services to qualifying patients who are uninsured or underinsured and have income under 200% of the Federal Poverty Level (FPL) based on their household size. The FPL is set by the federal government each year and qualifies patients who may receive discounted care. Income levels are based on a patient’s annual household income before taxes are taken out and the size of their household.

Income is all earned income from all members in a household, excluding non-cash benefits such as food stamps and housing subsidies.

Household family size includes all individuals living in the same residence who are related by birth, adoption, or marriage. Household family size also includes unrelated family members living in the same residence who are supported by or who are a supporting member of the family.

To qualify, you must complete our Financial Assistance and Sliding Fee Scale Application within 60 days of your visit. You may download a the application in English or Spanish. If you have trouble downloading these forms, call our front desk at 401-648-4700 to receive a copy of the application. Our staff can help you complete this application for financial assistance. Along with the completed application, you must submit proof of income using at least one of the documentation types below:

  • Most recent W-2, 1099 or IRS tax return (1040EZ, etc.);
  • Paystubs, for the past 90 days;
  • Signed letter from your employer stating cash earnings ;
  • Self-distribution statements for Business owners or a letter stating how much you usually earn in a month or year;
  • Unemployment or disability check stubs for 90 days and/or letter detailing unemployment benefits from your state unemployment office;
  • Military leave and earnings statement;
  • Proof of all other income (SSI, SSDI, pensions, etc.);
  • Proof of monthly living expenses such as rent, car payments, and insurance receipts, etc.;
  • Proof of outstanding debt (copies of bills/statements that show recent payments);
  • Application forms and/or statements for government and/or other sponsored assistance;
  • Notarized letter stating you have no source of income, signed by you;
  • Mock modified adjusted gross income worksheet IF none of the above are available to you (ask Open Door Health staff for assistance if this is required).
  • **Bank statements do not qualify as proof of income.*

Patients living with HIV are eligible for our Ryan White Financial Assistance Sliding Fee Scale. The Ryan White Program is a federal program that helps us cover some health care costs for patients with HIV. The Ryan White Financial Assistance Sliding Fee Scale is almost identical to the Open Door Health Financial Assistance and Sliding Fee Scale for patients who are not living with HIV. However, patients with HIV may not be eligible for this sliding fee scale program if they are eligible for another benefit that would pay for their care at Open Door Health (e.g. Medicaid). Patients must have proof of their HIV diagnosis on file at Open Door Health to qualify for the Ryan White Financial Assistance Sliding Fee Scale.

Additionally, the federal government sets a limit on what someone living with HIV can spend on their health care costs at agencies that accept Ryan White funding. Based on these limits, and taking into consideration a person’s gross annual income, Open Door Health has set a maximum amount a patient can be charged directly for health care services in a calendar year (January 1 through December 31).
